How exactly does one become banned and is it completely up to the mod's feelings rather than breaking of rules or general annoyance?

i've been banned before.  so, i have some perspective.  but, before i say anything let me state i am only speaking from my perspective and my own views on the matter in retrospect.
first, as far as i know it has to be a unanimous decision amongst the titans.  and i see the titans as being a large slice of different opinions and personality types.  it would be like trying to get 18 different cats to agree on something, if you follow my meaning.
secondly, and here i get very theoritical, but i really feel this to be true;  ppl get banned when they are causing other ppl harm.  and what i realized is this.  not everyone is strong.  i am.  you are.  those that post with passion are.  but some ppl are hurt by well meant posts.  (or badly intended, who am i to judge?)
point is this.  if you are a strong person you have to be careful with your strength so that those who might not understand you are not dealt harshly.
when i was banned, i truly believe the titans meant well.  i was enganged in activities that was harmful to others.  and i don't think there is a conspiracy, or a popularity contest, or any sort of fascism.
i was allowed to return.  and i have been much more careful in how i treat others.  it is about noblisse oblige, i think.
(oops edit!  i just noticed you said question for the mods!  hope i'm not sticking my neck out intrusively!   i don't think just the mods are involved, there are admins, mods, and titans.  of which i am none.  but i hope my thoughts help.)

If this was truly the case, there would be a lot more banned people.

or...it is hard to judge, hard to come to a consensus, and i'm glad i'm not in that position of authority.
